Henry: Portrait of a Serial Killer is far and away the best serial killer movie I've ever seen. It's loosely based on the exploits of Henry Lee Lucas, and I mean loosely, but it's one of the dreariest, low energy horror movies I've ever seen. And that makes it all the more gruesome. When the credits roll, you'll feel sick.
It is seriously among the most effective horror movies I've ever seen.
I don't know what sort of availability it has on Region 1 DVD, but I bought an R2 a little while ago for dirt cheap on ebay. Even after the exchange from dollars to pounds, it was cheaper than a US disc.

I rather like the odd serial killer films, which Henry: Portrait of a Serial Killer is very much a part of. I'd also say the serial killer films WITHOUT cops or crazy plots like Seven are some of the best ones. Here is a list of serial killer films most people never watch:
Michael Powell's Peeping Tom
Schramm
Scrapbook
The Vanishing
Driller Killer
Ms.45
Man Bites Dog
From the Life of the Marionettes and a few others
Also just remembered:
Angst
FAMILY PORTRAITS: A TRILOGY OF AMERICA

Driller Killer
Ms.45
Man Bites Dog

Does Arsenic and Old Lace count?
Cary Grant goes to visit his elderly aunts. They poison their gentleman callers with Elderberry wine. And their brother (who thinks He's Teddy Roosevelt, and is digging the panama canal in the basement), buries the bodies in the basement of the house. Hilarity ensues.
May 10, 2005. It was the day the music died. David Wayne Rest in Peace.
I'll second the Schramm recommendation, but I'm a big fan of all Jorg Buttgereit's movies.
Although I did like it, I agree with Kimberly that Henry seems a bit overrated to me.
I really liked Saw also and M of course set the standard.
Normally these types of questions seem silly to me, but to be honest, I'm actually having a real hard time coming up with some serial killer films that really stood out to me. And I'm really trying to think of pure SERIAL killer films and not slashers (ie Halloween style movies) or psycho killers (ie Luther the Geek).

The only one that stands out to me is Hunting Humans. It's pretty low budget and I thought the story could have used a few minor adjustments but, the premise was pretty cool and that's what made me end up liking it quite a bit.
It's about a serial killer that stalks victims but, another serial killer is stalking him and beats him to the punch by killing the people the other guy is stalking before he can do it himself.

I too am very fond of american psycho, I dont know I just love the patrik bateman character. I really love the part were hes talking about Genesis just before he kills that one chick, he states his dislike for the early albums cause they were too "artsy", lol, loved it!

Yeah, I saw that one. It stars that fat guy from Pee Wee's Big Adventure as Gacy. It was pretty good. Ed Gein was a pretty cool movie too, but unfortunately most serial killer films I see are either low budget or made for TV. Ted Bundy's had a couple of made for tv movies and both have sucked ass I thought. Henry was cool, although the writers took a lot of liberties with the script but that's Hollywood.
Did anyone see the Gacy made for TV movie starring Brian Dennehy? That was okay from what I remember........

Deranged, about Ed Gein is pretty cool (one of Savini's first gore FX films)
Exorcist 3, though Demon powered was a great serial Killer movie(Gemini Killer-inspired by the Zodiac Killer)
